Susamaru was once a young girl noted for her carefree attitude and her skill at handball. Everything changed when she caught the attention of Muzan Kibutsuji, the first demon, who turned her into one of his principal assassins and implanted her with a desire to become one of the Twelve Kizuki. Despite the fiercely combative and territorial nature of demons, Susamaru formed a close bond with the demon Yahaba, and the two became close partners, hunting down various targets at Muzan's request and using their powers in sync to pose a unique threat.
Upon the arrival of Tanjiro Kamado in Asakusa, Muzan, disgusted at the appearance of a Demon Slayer, dispatched Susamaru and Yahaba to make an example of him. Tracking him to the home of rogue demon Tamayo and her follower Yushiro, the duo assaulted the house, destroying it and forcing Tanjiro and his demon sister Nezuko to ally themselves with the other demons to survive. After a vicious fight, an impressed Susamaru prepared to battle Nezuko further, but was tricked by Tamayo into uttering Muzan's name out loud, activating the curse placed on her and horrifically killing her. Reduced to a spirit, Susamaru is given one of her mari balls by Tanjiro, who laments that someone so young could be manipulated into darkness before watching her remains disintegrate.
